Fostering vibrant communities where residents feel supported and empowered is at the root of CONNECT. The simple goal is to increase housing stability and resident well-being through a holistic case management approach that creates places of pride and belonging.

CONNECT is rooted in Housing Visions’ mission to be the catalyst for sustainable positive change in neighborhoods. We strive to foster environments where residents have a place to call home and the resources and support necessary to thrive. CONNECT offers an array of services to meet the needs of our residents.

CONNECT’s Strategy

CONNECT’s strategy is employed through a non-judgmental, person-centered, and trauma-informed lens, promoting overall stability and well-being.

The CONNECT framework consists of the following:

Case management: Our skilled case managers provide supportive counseling and personalized support to residents, assisting them in identifying their strengths, setting and achieving goals, and overcoming challenges.

Outreach: We engage with residents proactively, reaching out to guarantee that everyone has access to the support and services they need to thrive. The CONNECT team stays up to date on community events and resources and provides referrals to trusted service providers tailored to the unique needs of each resident.

Navigation: We help residents navigate complex systems, ensuring they can access quality food, healthcare, public benefits, education, employment opportunities, and more.

Networking: Collaboration is at the heart of our approach. The CONNECT team partners with providers to bring community services on-site to offer workshops and support services. We interact with residents, community partners, and stakeholders to establish and maintain connections with community providers to help our residents maintain their housing stability.

Empowerment: Through education, skill-building, and advocacy, we empower residents to take control of their lives and futures. We strive to build strength and independence, encouraging residents to become their own advocates.

Community: Strong communities are built on connections. Through a variety of activities, we foster a sense of belonging and mutual support among residents. From educational workshops and social events to volunteer opportunities and support groups, residents have ample opportunities to engage, connect, and grow together.

Trust: Resident longevity is important to build their social network and cultivate bonds with their neighbors, improving the strength of their community. Using a trauma-informed approach, CONNECT aims to help facilitate the activities necessary to build trust, connections, and engagement to promote the well-being of residents and their communities.
